Chronic Care
Each patient is assigned the type(s) of chronic care they require such as diabetes, HIV, asthma, seizure, and hypertension. Chronic care clinics may be added or modified as required to fit the institution's needs. This allows you to ientify and track your chronic care population to ensure they are receiving treatment on a regular and timely basis. When used in multi-facility settings the system will provide continuity of care during patient transfers.
